If you're applying for CLTC or already approved, we can help you:

  • Understand your eligibility

  • Communicate with your CLTC case manager

  • Select GDHC as your home care provider

  • Start receiving approved services quickly

  • Maintain documentation for ongoing benefits

How do I start CLTC Services ?

Step 1 — Call CLTC Intake:
Ask for a Long-Term Care Assessment.

Step 2 — Complete your screening:
A nurse will evaluate your daily needs.

Step 3 — Choose your provider:
Ask your case manager to list Gracefully Diverse In Home Care.

Step 4 — Begin services:
We coordinate your schedule and caregiver introduction.

“How soon can you start services, and what does the first visit look like?”


“We can begin very quickly. The first step is a free in-home consultation, which is extremely informal and focused on comfort. During this visit, we simply get to know you, understand your routines, and talk through what kind of support would make your day-to-day life easier.

This visit can also include a free Nurse assessment to accurately determine what level of care is needed. Our goal is to make sure your needs can be safely and fully met by our non-medical home care staff. If your needs ever exceed the services we are legally able to provide, we will kindly help you find the right level of care and guide you toward the appropriate clinical or medical resources. We believe in doing what is best for you—even if that means helping you connect with another provider.

Once the assessment is complete, we can typically start services within a few days, depending on the schedule you prefer.”
